Birth Certificates
The Seneca County General Health District can now provide certified copies of birth certificates for any person born in the State of Ohio from 1908 to the present, regardless of the county of birth. This allows you to get your birth certificate without the inconvenience of traveling to your birth county or applying by mail. Birth records prior to 1908 must be obtained at the Seneca County Probate Court, located at 103 E. Market St, Suite 301, Tiffin, Ohio.
Death Certificates
Death Certificates for an individual who passed away in in the state of Ohio may be obtained from the Seneca County General Health District. Death records prior to 1908 must be obtained at the Seneca County Probate Court, located at 103 E. Market St., Suite 301, Tiffin, Ohio.
Certificate Fees
Certified birth or death certificates are $25 per copy. We accept cash, check, debit/credit card or money order. Debit/credit card purchases require an additional processing fee. Due to changes in state law, we can not provide uncertified copies of birth or death certificates.
How to Order
Online: Order HERE
In Person: Visit the Vital Statistics Office at the Seneca County General Health District. In most cases, your certificate will be processed and issued in about 15 minutes.
By Phone: Order by phone with a debit or credit card by calling 419-447-3691 ext. 6300. Certificates will be sent via the U.S. Postal Service.**
By Mail: Mail the completed Birth Certificate Request Form, a self-addressed stamped envelope, and a check or money order of $25.00 per copy to:

Upon receipt of your request, your certificate(s) will be mailed back to you within 2 business days. Please allow 7-10 business days for the return mail.**

Notice:
Effective October 15, 2015, the Ohio Revised Code (ORC) has changed requiring social security numbers on death certificates to provide proof of why he/she is entitled to such information. This applies only to the first 5 years following the date of death.
