Community Health Status Assessment
The Community Health Status Assessment is a comprehensive and actionable picture of health and well-being in Seneca County. The assessment is conducted every three years.
The purpose of the CHSA is to:
- Analyze Seneca County health trends
- Identify and prioritize health needs
- Inform identification of priorities in the Community Health Improvement Plan (CHIP)
- Provide a template for agencies and local partners with a uniform set of categories and metrics to use in related assessments.
