Immunizations

Vaccine-preventable disease levels in the United States are at or near record lows. However, this is not the case around the world. Even though most infants and toddlers in the United States have received all recommended vaccines by age two, many under-immunized children remain, creating the possibility of disease outbreaks.

Children who have not had all their infant immunizations are at risk for getting these diseases from the people who care for them. Many teens and adults are missing the chance to protect not only themselves, but also children under the age of two against deadly diseases such as Hepatitis B, Pertussis (Whooping Cough), influenza, and pneumococcal disease.

Many private insurances, Medicaid and private payments (cash, check, credit & debit cards)  are accepted for Immunization Services. There is an additional processing fee for credit and debit cards. You are responsible to know what facilities/services are covered by your insurance agency. Please call if you have any questions.

Consent to Treat for Immunizations

The Immunization Consent Form for Children and the HIPAA Consent Form must be completed by the parent or legal guardian if a friend or family member will be bringing your child for their immunization appointment at the Seneca County General Health District.

Send the completed forms, a current shot record, and health insurance card(s) with the adult who is bringing your child to the appointment.

Medical Record Request Form

Medical records are available for $10 per record. Cash, checks, and credit or debit cards (with an additional processing fee) are accepted. A state photo ID is required.

Walk-in vaccine clinics will be held from 10:30 a.m. to 3:30 p.m. on June 9, June 11, June 18, June 23, and June 25 at 92 E. Perry Street in Tiffin. Please bring a photo ID and insurance card. Fostoria student vaccine clinic announcement
Fostoria area students can receive recommended vaccinations at upcoming vaccine clinics hosted by the Seneca County General Health District. Clinics will be held from 9:00 a.m. to 2:00 p.m. on July 21, August 18, and September 15 at 801 Kirk Street in Fostoria. Appointments are recommended, but walk-ins are welcome while supplies last.
